Who can join?

  • 1. 18 years old and above
  • 2. With capacity to contract
  • 3. With work, business or any legal source of income
  • 4. Permanently residing or a homeowner, regular employee of a company within Cavite
      or has a legal business within Cavite (to be considered as a Regular Member)
  • 5. Renting a house or tenant in Cavite, regular employee of a company within
     Cavite or has a legal business within Cavite (to be considered as an Associate Member)
  • 6. Residing within Cavite with regular pension
     (to be considered as an Associate Pensioner Member)

Membership Application Requirements:

  • 1. Accomplished membership form.
  • 2. Proof of billing
  • 3. TIN NUMBER
    Additional requirements if NO TIN:
    1 Singed PSA
    2 Completely filled out BIR Form 1904
    3 Photocopy of 2 Valid Government-issued IDs showing the NAME
    ADDRESS and BIRTHDATE of the bearer - with 3 specimen
    signatures
    4 4Birth Certificate (for SINGLE applicants)
    Marriage Contract (for MARRIED applicants)
    5 Php 100 Notarial Fee
  • 4. Barangay Clearance
  • 5. Photocopy of any valid ID (preferably government-issued IDs)
  • 6. Barangay Business Permit / Mayor’s Permit (for business owners)
  • 7. Must attend Pre-Membership Seminar (every MWF, 1PM at DACCO Main Office)
  • 8. Subject for background investigation
  • 9. Must pay the following fees:
    •  MEMBERSHIP FEE = P300.00
     ID FEE = P50.00
     INITIAL SHARE CAPITAL = P1,000.00
     DKKM = P250.00 (Optional for 18-64 years old; REQUIRED for 65 years old and above)
     Total = P 1,350.00 OR P1,600(if with DKM)
